Athletic Forms
ALL of the following forms MUST be completed, signed and on file in the PHS Athletic Office before participating:
Physicals for the 2023-2024 School Year must be dated after 5/1/2023.
Please note the physical packet changed last year.
There are now 2 places the medical provider must sign and
student/guardian signatures have changed as well.
Please look over the packet carefully.
All students who participate in athletics are required to have insurance. If you do not have a family insurance plan or would like a supplemental insurance to the plan you have please click the following link or see Dannielle in the Athletic Office.
Supplemental Insurance - administered by Student Assurance Services, Inc.
